The Caribbean Public Health Agency (CARPHA) has received financing from the World Bank toward the cost of the OECS Regional Health Project (P168539). CARPHA intends to apply part of the proceeds of this financing toward payments under the contract Supply and Delivery of Personal Protective Equipment (PPE)
You are hereby invited to submit your quotation for the supply and delivery of laboratory reagents and consumables, as detailed in the Request for Quotation (RfQ) document.
In preparing your quotation, please ensure that you:

Review all documents contained in the Tender package.

Complete the Form of Quotation (Page 9 in Tender Package) and the Terms of Conditions of Supply (Page 10-11 in Tender Package).

Review and complete the PPE Technical Specification with the specifications offered.

Review and sign the World Bank’s Letter of Acceptance of the Anticorruption Guidelines and Sanctions Framework; and provide all required supporting documentation as specified in the Tender package.
For your reference, the World Bank Form of Contract Agreement is also included in the Tender package.
